Please set aside approximately 20 minutes to complete the online intake form prior to your first appointment.
I appreciate the time you take to complete the intake form, as it provides valuable information to me about how best to treat you. Completing the form prior to your appointment allows me to review it before you arrive. However, if you are unable to complete the intake paperwork before your appointment, no worries – I’m still happy to see you!
If you have questions after reading through the forms, please feel free to call or email me, or you can ask during your appointment.

Ying Spring Acupuncture is not an in-network provider with any insurance companies at this time, and I do not bill insurance (health, worker’s compensation, or auto) for services provided. Ying Spring Acupuncture can provide a receipt for you to submit to insurance, however this does not guarantee that you will be reimbursed by your health insurance company.
A 24-hour notice is required to cancel or reschedule an appointment. Use the link in your confirmation email to make any changes to your appointment or call the clinic at 612-305-8089 at least 24-hours prior to your appointment time. Because a time slot is reserved especially for you, appointments that are rescheduled or canceled with less than a 24-hour notice will be charged the full appointment fee to the card on file. Exceptions will be made on a case-by-case basis for medical or family emergencies or when Minneapolis or St. Paul Public Schools are closed due to inclement weather.
If you are running late, please contact the clinic at 612-305-8089.
If you arrive 15 minutes after the appointment start-time without advance notice, the appointment will be considered canceled and you will be charged the full appointment fee to the card on file.
